Preoperative evaluation of EDTA-dependent pseudothrombocytopenia before lumbar disc herniation surgery: A Case Report

Ethylenediaminetetraacetic acid dependent pseudothrombocytopenia (EDTA-PTCP); is an invitro phenomenon characterized by the use of EDTA as an anticoagulant, the clustering of platelets leading decreased platelet count by the analyzer in complete blood count. In this case report, we present a 47 years old male patient admitted to neurosurgery clinic with complaints of back and leg pain. Thrombocytopenia was detected in preoperative testing of patient who had been decided surgical operation after neurological examination and lumbar magnetic resonance imaging (MR). Complete blood count was reanalysed using citrate according to EDTA and peripheral blood smear of the patient was examined. Thrombocytopenia was not detected with citrated sample in complete blood count and platelet clusters were not detected in peripheral smear using citrated blood. Pseudothrombocytopenia was considered in the patient whose anamnesis, physical examination findings and coagulation tests were incompatible with thrombocytopenia. In conclusion, EDTA-PTCP was diagnosed in preoperative evaluation of patient.
